Neighbourly Podcast Guest Release Form

We appreciate your willingness to share your story with Neighbourly, CareImpact, and its subsidiaries (the “Producer”). Your story is important and valuable, and we want to ensure that you are comfortable with how it may be used.

By agreeing to the following statements, you give the Producer permission to use your story in whole or in part, but only in a manner that respects your dignity, privacy, and intent. You have the right to revoke this permission at any time, with the understanding that the Producer will make every reasonable effort to remove the content from its online platforms.

1. Ownership and Permission

I understand that my story is my own, and I am granting permission for the Producer to record, use, and share it in a respectful and ethical manner.

2. Editing and Sharing

I understand that my interview may be edited for clarity and length, and shared both now and in the future with the organization’s audiences through websites, podcasts, videos, training, and educational opportunities.

3. Use in Other Projects

I understand that the Producer may use clips or segments of my interview in other CareImpact projects, such as training materials, awareness campaigns, and community engagement efforts, provided these uses align with the organization’s mission and respect my dignity and intent.

4. Public Online Forums

I understand that my story may be shared on Neighbourly, Neighbourly Headlines, and CareImpact’s public online forums (email, social media, newsletters, etc.).
If the Producer wishes to share my story outside of CareImpact platforms, they will first obtain my explicit permission.

5. Integrity and Respect

I understand that the Producer is committed to preserving the integrity of my story, and will only use it in ways that are respectful and do not harm or exploit me.

6. Privacy and Safety

I understand that any media shared online is publicly accessible, and while the Producer will make every effort to protect my privacy and safety, complete confidentiality cannot be guaranteed.

7. Monetary Aspects

I understand that the podcast may include ad campaigns, sponsorships, or promotions to support CareImpact’s mission and cover production costs, but the Producer will not profit directly from my story without my explicit consent.

Final Assurances
The Producer is committed to using your story in a way that is respectful, fair, and just. We will not exploit your story, and we will make every effort to ensure you feel safe and supported throughout the storytelling process.
