*Guest Release Agreement

We appreciate your willingness to share your story with Journey With Care, CareImpact, and its subsidiaries (the “Producer”). Your story is important and valuable, and we want to ensure that you are comfortable with how it may be used.
By agreeing to the following statements, you give the Producer permission to use your story in whole or in part, but only in a manner that respects your dignity and autonomy. You have the right to revoke this permission at any time with the knowledge that the Producer will make a considered effort to remove the content from its online platforms.


1. Ownership and Permission

  • I understand that my story is my own, and I am granting permission for the Producer to use it in a respectful and ethical manner.

2. Editing and Sharing

  • I understand that the information I provide may be edited and shared both immediately and in the future with the organization’s audiences on websites, videos, and educational opportunities.

3. Use in Other Projects

  • I understand that the Producer may use parts of my interview, including clips and segments, for other CareImpact projects, podcasts, or related initiatives, such as training materials, awareness campaigns, and community engagement efforts, provided that these uses align with the mission of the organization and respect my dignity and intent.

4. Public Online Forums

  • I understand that the information I provide may be used on Journey With Care and CareImpact’s public online forums (email, social media, newsletters, podcasts, etc.).

  • I understand that the information I provide may be used on other public online forums (email, social media, newsletters, podcasts, etc.), but only with my explicit permission.

5. Integrity and Respect

  • I understand that the Producer may use any details they have obtained to preserve the integrity of the story, but only in a way that is respectful and does not harm or exploit me.

6. Privacy and Safety

  • I understand that any social media invitations shared on the internet are subject to sharing and accessible globally, and that the Producer will make every effort to protect my privacy and safety.

7. Monetary Aspects

  • I understand that the podcast may or may not run ad campaigns, sponsorships, or other promotions to support and further the work of CareImpact and cover the costs associated with the podcast, but the Producer will not profit from my story without my explicit consent.


We want to assure you that the Producer is committed to using your story in a way that is respectful, fair, and just. We will not exploit or profit from your story without your explicit consent, and we will make every effort to ensure that you feel safe and supported throughout the storytelling process.
